The pop star, who has booked all 13 rooms of the luxury Kumbali Country Lodge for herself and her entourage, hopes to adopt a four-year-old orphan called Mercy James.
But the children’s charity Save the Children has urged the singer to think twice before giving a home to a second African child.
Spokesman for the charity Dominic Nutt told Sky News the adoption of a child from an orphanage was a bad idea.
He said: "We believe they are better looked after in their own country as far as is possible."
Mr Nutt suggested the child’s extended family were better placed to look after the girl.
He added: "International adoption can actually exacerbate the problem it hopes to solve."
Madonna’s spokeswoman Liz Rosenberg, who has not commented on the adoption reports, said the star would not respond to Save the Children.
Questions were asked after Madonna adopted her first child David Banda with her then husband Guy Ritchie.
The pair took the Malawian child into their London home in 2006 when he was 13 months old. Their quest for a sister for him has sparked renewed criticism.
Mercy’s grandmother has hit out at the Material Girl star for her adoption plans.
Lucy Chekechiwa, 61, told The Sun: "Why doesn’t this singer pick other children? It is stealing. I want to go to court, I won’t let her go."
Sources say Mercy’s mother, an unmarried 18-year-old, died months after she was born.
Her father is believed to be alive but no other details are publicly known.
